We've put together a library of resources that are just a click away!  Theses materials have been designed especially for our families in Hawai'i to assist them in navigating the system.  Hawai'i received one of the first New Freedom Initiative Integrated Service System Grants for Children with Special Health Care Needs. We called it the Hilopa'a Project.  The materials below were developed through that grant.  If you are looking for other health related information - do not hesitate, contact us directly. 
 

The Rainbow Book is a Medical Home Resource Guide for Children and Youth with Special Health Care Needs.  Both the Hilopa'a Project and the Hilopa'a F2FHIC offer Rainbow Book training.  Please see our training page for our next scheduled training.  We've created an online table of contents of the book, for you to "launch" off to navigate through the internet, all of the programs and services that are found in the book. 
 

Our Personal Health Record was developed by the Hilopa'a Project.  It is an integrated portable medical record summary for pediatric and adult medicine, that could last a life time! This workbook is meant to be a guide to help “talk story” and talk through the planning process of transitioning our youth to adult health. As our mentor Josie Woll has always taught us, life is a journey, not an event. Transitioning to adulthood does not happen overnight.  Successful transition requires time, patience, and a sense of humor.
